This form is to be used whenever a Scout is planning on being more than one hour late for or leaving early from

a scheduled Scouting activity such as a campout. The purpose is:

     1. For the Patrol Leader to plan duty rosters and activities accordingly;

     2. To ensure that Leadership accounts for all Scouts at all time during an activity;

     3. Persons other than parents/guardians are authorized to pick up or drop off a Scout.

     4. To help ensure the overall safety of the Scout.

This form MUST be completely filled out and handed to the

Scoutmaster/Activity Leader prior to the event.
